Tower Hamlets PCT and its partners are working together with Marie Curie Delivering Choice Programme to improve the choices of treament and place of care option available to service users and their families.

The Care-Plus Project in Tower Hamlets scheme hopes to cut emergency admissions and acute bed days.

Co-ordinators have been appointed to supoprt carers of patients with end stage heart failure, additionally this aims to ensure more patients are able to choose where they die becuase their carers are well supported.
